What is Social Prescribing?

Social Prescribing is a service that GPs, nurses and other healthcare professionals can refer into for patients who need extra services and support for their health and well-being.

The service offers one-to-one sessions where people can talk about their worries and things that impact their health and get support to find the best solution.

Social Prescribing also helps healthcare professionals care for their patients because sometimes the factors influencing health are complex in nature and cannott always be fixed with a prescription for medication.
